Scoring Instructions:

This questionnaire provides three scores - one for depression, one for anxiety and one for stress.  To calculate your three scores add up your responses to the following items and then multiply by 2:

- Depression 3 + 5 + 10 + 13 + 16 + 17 + 21
- Anxiety 2 + 4 + 7 + 9 + 15 + 19 + 20
- Stress 1 + 6 + 8 + 11 + 12 + 14 + 18

What your score means

For depression:
0 - 9 = normal
10 - 13 = mild
14 - 20 = moderate
21 - 27 = severe
more than 28 = very severe

For anxiety:
0 - 7 = normal
8 - 9 = mild
10 - 14 = moderate
15 - 19 = severe
more than 20 = very severe

For stress:
0 - 4 = normal
15 - 18 = mild
19 - 25 = moderate
26 - 33 = severe
more than 34 = very severe
